On 10-13-16 I switched from Showtime to STARZ. I spoke to Kim. I was told that after 3 months I would be eligible for a $50.00 rebate (not $25.00). I cannot find the form
and would be real happy if you would mail one of them to me so I can apply for either $25 or $50 rebate as promised. I am a Verizon retired person & even though the STARTZ channels #340 to 344 has so many pixels it is impossible to watch them I’m not receiving proper clarity . I believe you will honor my request.
